The red lines we see on the screen when Nathan (or Chloe) gets hit is just his luck meter diminishing and the chances of him getting shot by enemy fire increasing.
Animator Jonathan Cooper revealed the news on Twitter, commenting that this was the most surprising thing he learned when joining the studio.
“That’s a mega-swing! Sidenote I learned on joining the team: Drake doesn’t ever take bullet damage. The red UI that shows ‘hits’ is to represent his ‘luck’ running out. Eventually, enemies will get a clear shot and kill him if he takes enough near-misses,” he said.

Series creator Amy Hennig, who we’ve already reported today because of her statements about making single-player games, confirmed that, again, on Twitter.
“True! That was the original intention (to stay more aligned with the spirit and tone of the films we were homaging),” hinting at the consonance the series wanted to have with movies like Indiana Jones and others.

So, this was quite surprising and even explains how is it possible that Nathan, who never wears a particular suit to defend himself, manages to survive this long when ‘hit’ by enemies. Now, we’ll need an explanation about how is it possible that enemies took this long to actually hit him…
